We are in-network with Aetna, BCBS, Itasca Medical Care (IM Care), Medica, Medicaid, Minnesota Medical Assistance, Medicare, Optum, TriCare/TriWest and United Healthcare.
We also offer sliding scale and private pay options.
Please note that any copays, deductibles, coinsurance, or other patient responsibility amounts are due at the time of service and will be charged to the card on file in accordance with your insurance benefits.

Cancellation and Missed Appointment Policy
We kindly request that patients provide at least 24 hours’ notice for any appointment cancellations or rescheduling. This allows us to offer the appointment time to other patients in need of care. Late cancellations (less than 24 hours’ notice) and missed appointments (no-shows) may be subject to a $75 fee, which will be charged to the card on file at the time of the missed appointment, depending on the patient’s insurance. This fee is the responsibility of the patient and may not be covered by insurance. We understand that emergencies and unforeseen circumstances can arise, and we encourage patients to contact our office as soon as possible if they are unable to attend their scheduled visit. Consistent attendance helps ensure continuity of care and the best possible treatment outcomes.
